an overall evaluation of your symptoms and signs. A diagnosis
will be made with suggested guidelines for treatment and dietary
recommendations.
Herbal
Consultation and Herbal Prescription

Includes
an overall evaluation of your symptoms and signs, using Chinese
Medicine's method of tongue diagnosis and pulse diagnosis. Based
on the root cause of the imbalances, a customized herbal prescription
will be written for you. The herbs, like prescription medicine,
are
an additional fee.
Herbal
Consultation: Follow-up

Follow
ups are necessary because in the eyes of Chinese Medicine your
condition changes day to day. You may have Heat with your system
now but after treatment with the herbs, the Heat may have dissipated.
Then your herbs must change accordingly because those same herbs
are no longer right for your body.
Herbs

Costs vary depending on the type and form of herbs: powders, pills,
or raw herbs.
Customized
herbs are ordered and shipped directly to you. Herbs typically
last for a period of 2-3 weeks, sometimes longer. But people's
conditions can change from week to week. Weekly follow-ups are
recommended at least in the beginning.

types of services. Visa & Mastercard accepted.
Forms
of Payment
Cash, check, and credit cards (Visa, Mastercard) are accepted. Payment
plans are also available.
Cancellation
Policy
Please note there is a minimum of 24 hours notice for cancellations
to avoid being charged for your appointment.
Insurance
It is highly recommended to call your insurance company to verify
if your policy covers acupuncture services. Ask (1) how many visits
per year you are allowed, and (2) how much per visit you will be
reimbursed.
We
do not accept insurance, but we do provide patients with "Super-Bills".
These "Super Bill" forms are an easy way for you to be
reimbursed from your insurance companies for the acupuncture services
you have already paid.
We
give you this form and you simply fill in your policy number and
mail it. The reimbursment will be sent to you directly. Upon request,
we can electronically submit these Super Bills to your insurance
companies on your behalf. The reimbursement check from the insurance
company will be mailed directly to you.
